Application for Chainsaw Registration

 

Requirements

Original/New Application

Holder of a subsisting Tenurial Instrument such as Timber License Agreement, Production Sharing Agreement, Co-production Sharing Agreement, or a Private Land Timber Permit/Special Private Land Timber Permit, CBFMA, IFMA, SIFMA, or other similar Tenurial instruments:

  1. Duly accomplished application form
  2. Copy of the Tenurial Instrument
  3. Proof of the Detailed specification of chainsaws to be registered
    1. Brand, Model, Engine Capacity, Serial Number
    2. Purpose of Use
    3. Area/Location the chainsaw will be used
    4. Name of Owner
    5. Date of Purchase
    6. Name of Dealer
  4. Proof of ownership of chainsawRegistration fee
    1. Sales Invoice, Deed of Sale, etc.
  5. Registration fee

Orchard/Fruit Tree Farmer or Industrial Tree Farmer:

  1. Duly accomplished application form
  2. Proof of the Detailed specification of chainsaws to be registered
    1. Brand, Model, Engine Capacity, Serial Number
    2. Purpose of Use
    3. Area/Location the chainsaw will be used
    4. Name of Owner
    5. Date of Purchase
    6. Name of Dealer
  3. Proof of ownership of chainsawCopy of Certificate of Tree Plantation Ownership (CTPO)
    1. Sales Invoice, Deed of Sale, etc.
  4. Copy of Certificate of Tree Plantation Ownership (CTPO)
  5. Certification from the concerned Barangay Captain, that the applicant is an orchard or tree farmer
  6. Registration fee

Licensed Wood Processors

  1. Duly accomplished application form
  2. Copy of the approved Wood Processing Plan Permit (WPPP)
  3. Proof of the Detailed specification of chainsaws to be registered
    1. Brand, Model, Engine Capacity, Serial Number
    2. Purpose of Use
    3. Area/Location the chainsaw will be used
    4. Name of Owner
    5. Date of Purchase
    6. Name of Dealer
  4. Proof of ownership of chainsaw
    1. Sales Invoice, Deed of Sale, etc.
  5. Registration fee

Agencies of the government, government-owned and controlled corporations

  1. Duly accomplished application form
  2. Certification from the Head of Office or his/her authorized representative, that the chainsaws are owned/possessed by the office and will be used for legal purpose
    1. Specify the purpose of use
  3. Proof of the Detailed specification of chainsaws to be registered
    1. Brand, Model, Engine Capacity, Serial Number
    2. Purpose of Use
    3. Area/Location the chainsaw will be used
    4. Name of Owner
    5. Date of Purchase
    6. Name of Dealer
  4. Registration fee

Other persons/entities who can show satisfactory proof that the possession and/or use of chainsaws is for legal purpose:

  1. Duly accomplished application form
  2. Copy of the Business Permit from the Local Government Unit or Affidavit that the chainsaw is needed in the applicant’s work/profession; and will be used for legal purpose
  3. Proof of the Detailed specification of chainsaws to be registered
    1. Brand, Model, Engine Capacity, Serial Number
    2. Purpose of Use
    3. Area/Location the chainsaw will be used
    4. Name of Owner
    5. Date of Purchase
    6. Name of Dealer
  4. Registration fee

Application for Renewal

1. Duly accomplished application form
2. Photocopy of the latest Certificate of Chainsaw Registration
3. Registration fee
